What Medicare Actually Covers — And What Most People Assume It Does

You worked hard for decades, paid into the system, and now Medicare is finally yours. It feels like a finish line. But here's the thing — a lot of people cross that finish line and find out the coverage they expected isn't quite what they got.

That gap between what Medicare does and what people think it does? That's where financial trouble often starts.


Let's Start With the Basics

Medicare is made up of four parts. Each one does something different.

Part A covers hospital stays. If you're admitted overnight, Part A is what kicks in. Most people don't pay a monthly premium for Part A if they worked long enough to earn it.

Part B covers outpatient care — doctor visits, lab work, preventive screenings, durable medical equipment. You do pay a monthly premium for Part B. In 2024, the standard amount is around $174 a month, though it can be higher depending on your income.

Part C is Medicare Advantage. This is an alternative way to get your Medicare benefits through a private insurance company instead of the government. We'll cover this more in another post.

Part D covers prescription drugs. It's sold through private insurers and added onto original Medicare.

So that's the foundation. Now let's talk about what's missing.


What Medicare Does NOT Cover

This is the part most people don't find out until they need it.

Dental. Original Medicare does not cover routine dental care. No cleanings, no fillings, no dentures. If you need dental work in retirement, you're paying for it yourself unless you have separate coverage.

Vision. Routine eye exams and eyeglasses are not covered. Medicare will cover eye care related to a medical condition — like glaucoma treatment — but a standard vision checkup and a new pair of glasses are on you.

Hearing. Hearing aids are not covered. Neither are routine hearing exams. For something that affects tens of millions of older Americans, this is a significant gap.

Long-term care. This one surprises people the most. Medicare does not cover nursing home stays beyond a limited window, and it does not cover in-home caregiving if you need help with daily activities long-term. This is one of the largest and most expensive gaps in the entire program.

Income replacement. If you get seriously ill and can't work or spend down savings to pay for care, Medicare doesn't replace the income you lost. It pays some of the medical bills. The rest is your problem.


Why the Gaps Matter Before You Enroll

Knowing Medicare exists is not the same as understanding what your Medicare actually does.

When people enroll without understanding the gaps, they often end up in the wrong plan — one that felt affordable on paper but leaves them exposed when something serious happens.

The time to learn this is before you sign anything. Once you're enrolled, changes can be limited to specific windows each year. And some decisions, like missing your enrollment window in the first place, can follow you with permanent cost penalties.
